Appointments:                     To schedule an appointment, please call (480) 272-0356 and leave a message with your name, telephone number, and say that you want to make an appointment. You can also contact me through email just be sure to include a contact phone number.  All appointments are scheduled after a brief telephone conversation.  Initial appointments cannot be scheduled via e-mail.

Fees are discussed at the time you contact me or when a referral is made.  If you have private insurance, PSYCH|assessment resources, PLLC, provides you with an invoice after payment is made and you can submit the invoice for reimbursement but cannot guarantee reimbursement of all or part of the fee. 

Fees for services delivered to other professionals (i.e., attorneys) or institutions are discussed at the time of the referral. 

Please contact your insurance company for details on benefits for ‘out of network’ providers.  Reduced fee services are available on a limited basis.  Payment is done by check or cash.
